As a clinician/researcher, you may need to complete the questionnaire on behalf of your Records/ Patients. Here, we will show you the step-by-step guide on how to do so:

Instructions to complete a questionnaire for your Participant/Patient

Step 1: Find your Participant

• Firstly, navigate to the Home Page and click on Participant dashboard.
• This will redirect you to the Program dashboard with a list of participants enrolled.

Step 2: Click on your Participant's name

• You can then click on the respective Participant's name, which is hyperlinked to Participant's details page, as seen in the below image.
• For instance, if you want to complete questionnaires for your patients, you would then click on their name.

Step 3: Select the Record ID then View Schedule

• On the Participant's details page, select RecordID from the drop-down menu.
• Then click on the View Schedule button on top of the page.

Step 4: Start the questionnaire

• A sidebar with all the Engagement details will pop up on the right.
• You can now click on the Fill in button to start the questionnaire.

Step 5: Complete the questionnaire

• You will be further redirected to the WeGuide Portal, with a list of all the tasks assigned.
• You can now click on Start to begin the survey and eventually complete it.
• The name of the Survey/Engagement is in bold, as seen in the figure below.
• You can see the name of the Participant that it relates to. 

As Clinicians often have multiple Patients, make sure you pay attention to the name of the Participant/Patient, to ensure that you are completing the correct questionnaire for the correct .
